| GAME REPORTS FOR ALL LEAGUES
MUST BE completely, and accurately filled out. The blanks are for required information. If other information is necessary, either write it on the front or back of the report, or on a separate sheet of paper. Better too much, than too little, information. If you have any questions, contact your assignor BEFORE submitting your game report to the league office. When you need to submit additional select game information, use the Referee Supplementary Report form via the link at the bottom of this page. Write accurately and concisely. You MUST note on the front of the select game report that the supplementary report is included. Keep a copy of both reports for your records. It is the responsibility of the referee crew, not just the referee, to make sure the report is fully and accurately completed. Each referee who neglects this paperwork requirement may lose games (be unassigned) and/or not receive game assignments for a period of time. Kids who are not checked in, and allowed to enter the field of play are guilty of illegal entry. The entire referee crew is at fault for allowing the illegal entry. If you are a mentor at the game, you have a responsibility to make sure the referee crew correctly and fully completes the game report. If a name is on the game report, you must clearly indicate whether the person was present and ready to participate (a player, coach, assistant coach), present and not participating (broken ankle, sitting out a suspension), or absent. If a player will arrive late, you must decide when that player may be checked in, and which referee will perform the check in. You must also verify that the coach, assistant coach have a valid risk management number. If you are a certified USSF mentor with GVSOA, it is a select game, and you work with the crew for the entire game, print your name and "mentor" on the report under the third referee's name.
